Annual Health & Medical Records
BSA requires anyone (including adults) that are attending local tours or weekend camping trips with Pack 1 to complete the annual health & medical record form Part A & B. This is to ensure your safety and well being.
Form Part A & B can be downloaded and submitted to Pack 1 through our online submission form.
Required Annually
Submission is required once per scouting program year and must be submitted prior to attending any local tour or camping trip.
